LiminalEase will act as a solid foundation for the client and their support team. LiminalEase helps create a blue print to building a bridge of communication. Some of the services LiminalEase can provide is the following: transition plan development, communication, therapeutic support, "All About Me Book", and oversight during the transitional stage.
Transitional Therapy Services
The therapist provides direct support to the adult and family. The therapist’s role is to assess the client’s skills, provide detailed assessments when requested, develop a plan of action, work with the adult on coping techniques, skill development and help with the transition stage by providing direct support.
"All About Me Book"
The “All About Me Book” consists of a detailed overview of the individual that can be shared with future caregivers, case managers, support staff, respite workers, and heath care professionals. The book is meant to be a living document that can be changed as a client grows and changes with time, but can be a consistent tool. It is the framework / bridge of communication that can be referenced and shared throughout their life.
LiminalEase will provide care giver training to future placements on the best methods to work with the client and do an overview of the “All About Me Book”.
Skill Development: In-Home or Community Based
LiminalEase can work on skill development in home or in the community. Skill development in home is the following, but not limited to the following: Activities of Daily Living, Cooking skills, House keeping skills, etc.
LiminalEase can also work on community inclusion skills of appropriate behavior in the community, discovering and using the local resources in near them, and/or work skill developemnt.

Development of Home Treatment Plan, Support, Behavior Support. Transition, Intervention
Technical assistance in carrying out the plan : Direct Support
LiminalEase will be a part of the entire process per the request of the client or caregiver. LiminalEase can provide direct transitional support by being present during the transition. The therapist can provide emotional and/or behavior support to the client as they transition. This will give them someone familiar to help them meet new care givers or support staff.
